How to Fill Out the Oregon DEQ Wood Stove Removal Notification Form (for Home Sellers)

To notify the Oregon Department of Environmental Quality (DEQ) about the removal of an uncertified wood stove as a home seller, follow these steps:

Access the Online Notification Form

Go to the DEQ Heat Smart submission page:
https://deqapps.oregon.gov/common/heatsmart/SubmitHome1

Step 1 – Contact Information

  • Select Home Seller or Buyer
  • Enter your name, address, phone number, and email
  • Click Next

Step 2 – Stove Location

  • Enter the address where the stove was located
  • Provide the tax lot number (found on your property tax statement)
  • Click Next

Step 3 – Stove Information

  • Enter details about the type of stove removed (e.g., wood stove, fireplace insert)
  • Click Next

Step 4 – Stove Disposal Information

  • Enter the name and address of the disposal facility
  • Include the receipt number for the disposal
  • Note: If SBC Recycle handled the woodstove recycling, we will provide you with all the necessary information to complete this step, including the facility name, address, and receipt number.
  • Click Next

Step 5 – Verify and Submit

  • Review your entries carefully
  • Make any needed corrections
  • Submit the form
  • Save or print the confirmation for your records
